MDS & Care Plan Coordinator
Position Summary
Mitchell County Hospital Health Systems is seeking an MDS & Care Plan Coordinator for their Resident Care Center. The successful candidate will be an RN with strong clinical and communication skills with a willingness to promote a positive environment for the residents and the care team. This position is responsible for comprehensive assessments, creating and updating individualized care plans, completing the MDS, and ensuring regulatory standard compliance. This position provides care to residents and assists the DON in managing day-to-day operations of the department. Full-time, 40 hours per week with an on-call rotation (every 2nd or 3rd weekend). Qualifications
●Associate’s or Bachelor’s degree in nursing from an accredited school of nursing
●RN with current licensure in the state of Kansas, RN experience strongly preferred
●Experience in geriatric nursing preferred
